In preparation for the coming school year, it is necessary to update student health records. This page contains information that will assist you in providing your child with any healthcare services that he/she might need in the coming year. Please click on the health forms link to look over this information, print out and return only the forms and information that pertains to your child.

Health History Record - Information in the Health History is Confidential. It will be maintained with the nurse. All students need to have a current health history on file. Please complete one for each child.

Administration of Medication Authorization - My license as a nurse allows me to administer medication to your child. But just as in the hospital setting, it requires a doctor's order. I also need to have parental consent. This applies to both prescription and over-the-counter medications. The Health Office does not stock any medications for students. Parents are responsible for supplying the nurse with any authorized medications. This authorization must be renewed each September.

NOTE: Please note some required grades changed for physical exams.

Physical Exams - Students are required to have a physical examination in grades K, 2, 4, 7 and 10. To meet New York State requirements, it has to be done within ninety (90) days of the start of school in September. Any student who does not bring in a physical within 15 days of the start of school will have one done by the school physician, Dr. R. Chasen, or one of his representatives from First Care in Monroe. Students who participate in sports need yearly physicals. A current physical is also needed when you apply for working papers.

Please mail forms directly back to me at the address below. This helps to assure that everything is properly documented for the new school year.
